Once your physical is complete, your child will be all set to attend school, play sports, and have a great year!
Requirements vary depending on your child's grade and whether they're participating in sports. Common requirements include a School Entry Health Exam as well as a Florida Certificate of Immunization (Form DH 680 or DH 681) for kindergarten entry and 7th grade.

What’s Typically Included in a Physical
