Requirements vary depending on your child's grade and whether they're participating in sports. Common requirements include a School Entry Health Exam as well as a Florida Certificate of Immunization (Form DH 680 or DH 681) for kindergarten entry and 7th grade.

Why Annual Physicals Matter
Annual physicals are an important part of keeping children healthy and active. These visits help your child’s provider track growth and development, catch potential concerns early, and make sure your child is ready for school, sports, or camp.
What’s Typically Included in a Physical
- A review of your child’s medical history
- Height, weight, and vital signs
- Vision and hearing screening (when required)
- A head-to-toe physical exam
- Review of immunization records
- Completion of required school, sports, or camp forms
Your child’s provider will also take time to answer any questions you have and share guidance based on your child’s age, activities, and overall health.

How do I book an appointment?
New patients can book online here or give us a call. Existing patients are welcome to use the portal to book (though you’re also always welcome to call!).

What are your office hours?
Our office hours vary slightly by location. We try to offer extended weekday hours as well as weekend availability to help meet your family’s needs. All our locations are open Monday through Friday with certain practices offering weekend hours You can see specific hours for each location here.
In addition, you can always call us to speak to our 24/7 on-call team.
Can I visit any Bluebird Kids location?
Yes, you are welcome to visit any of our locations! You can see the address and information for each location here.
I do not speak English. What other languages does your care team speak?
We care for many patients and families who do not speak English. Members of our care team speak Spanish and Hatian-Creole and anyone can access our live, real-time translator service when calling the office.
Can I bring all my children to appointments at the same time?
We do our best to accommodate parents and their schedules by seeing all children in the same family at or around the same time. We do recommend that you consider the ages and needs of your children and if having them at the same visit is best for them. For example, do you have a teen that does not want their care addressed in front of their younger siblings? Or children of different sexes who require a physical? You may want to bring another adult with you to help by staying in the waiting room with one or more kids while each child has their own visit.
What insurance plans do you accept?
We accept a wide range of insurance plans, including Medicaid. Our services are billed through your insurance. You will be responsible for the deductibles and co-pays outlined in your policy.

What’s the schedule for well-child visits?
We follow the American Academy of Pediatric’s recommended schedule: frequent early visits, then annual visits around each birthday. These visits include growth, developmental, and mental health screenings.

Should my child get their physical at a pediatrician's office or urgent care?
Urgent care can complete the forms, but it's not the same as having a pediatrician who knows your child. A physical at Bluebird Kids Health is a chance for your child's care team to get to know them: their growth, their history, and anything you've been wanting to ask. It's the kind of care that's harder to get when you're just passing through.
Similar to urgent care, we'll do our best to make booking the appointment as convenient as possible, offering same-day appointments whenever we can.
What's the difference between an annual check-up, a physical, and a sports physical?
Good news: They're all essentially the same visit. A check-up, a physical, a well-visit — these are just different names for your child's annual appointment with their pediatrician. During the visit, your care team will review growth and development, update vaccinations, complete any required screenings, and check in on how your child is doing overall.
If your child needs a form signed for school, sports, or camp, just let us know when you book. We'll make sure everything is taken care of.
How far in advance should I schedule my child's physical?
We recommend booking at least a few weeks ahead — especially in the summer, when back-to-school and sports physicals are in high demand. That said, if you're in a pinch, we offer same-day and next-day appointments and will always do our best to fit you in.
Do note that physicals are good for 365 days, so you don't need to wait until the summer to schedule.
What should my child bring to their physical?
Not much! If it's your child's first visit with us, it helps to bring any previous vaccination records or health history you have on hand. However, that's not at all required and we'll take care of it on our end. If your child needs a form completed for school, sports, or camp, bring that along too and we'll make sure it gets filled out on time.
My child's school or camp sent a specific form — can you complete it?
Yes. If your child's school, sport, or camp requires a specific form, just bring it to the appointment and we'll complete it for you. If you're not sure whether your child's upcoming visit covers everything the form requires, give us a call and we'll help you figure it out before you come in.
