In some cases, your child's provider may need to refer them to a specialist before their forms can be completed. If that happens, we'll let you know at the visit and help you figure out next steps.

We'll make sure you have the right forms, whether it's for kindergarten entry, 7th grade, or a sports team.

Start school

Once your physical is complete, your child will be all set to attend school, play sports, and have a great year!

What forms does my school require?

Requirements vary depending on your child's grade and whether they're participating in sports. Common requirements for kindergarten entry, 7th grade, and school sports include:

  • Form DH 3040: The School Entry Health Exam (also known as the yellow form)
  • Form DH 680: Florida Certificate of Immunization
  • EL2: Preparticipation Physical Evaluation (also known as FHSAA physical form)

When you book, just let us know your child's school and grade and we'll make sure we have everything we need.

Why Annual
Physicals Matter

Annual physicals are an important part of keeping children healthy and active. These visits help your child’s provider track growth and development, catch potential concerns early, and make sure your child is ready for school, sports, or camp.

What’s Typically Included in a Physical

  • A review of your child’s medical history
  • Height, weight, and vital signs
  • Vision and hearing screening (when required)
  • A head-to-toe physical exam
  • Review of immunization records
  • Completion of required school, sports, or camp forms

Your child’s provider will also take time to answer any questions you have and share guidance based on your child’s age, activities, and overall health.
